Job summary

Charis Hearts, Inc. is a statewide provider of in-home and community care through the TennCare CHOICES program. We collaborate with managed care organizations to deliver the highest level of care and support to clients in Tennessee.

We are looking for compassionate caregivers to join our team. Duties include non-medical in-home support, wellness checks, documentation, and adherence to individual service plans. Flexible shifts, top pay, benefits, and online training are available.

Qualifications

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.
• Strong communication skills – both oral and written.
• Reliable transportation to and from client locations.
• Compassionate and caring nature with a genuine desire to help others.
• Honesty and integrity in all interactions.
• Willingness to attend training sessions provided by referring agencies and/or the Tennessee Department of Mental Health & Developmental Disabilities (MHDD).

Responsibilities

• Caregivers provide non-medical in-home and/or community care based on the Client Service Plan.
• Perform client wellness checks to ensure their well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
• Provide non-medical support to agency service recipients.
• Be familiar with and follow all rules, policies, and procedures related to service delivery.
• Submit all required documentation accurately and on time.
• Adhere to the individual service plan for each recipient.
• Maintain strict confidentiality of all client records and information unless authorized by written consent.
• Immediately report any incidents of injury to your supervisor; report suspected abuse or neglect to authorities and supervisor.

Caregiver Responsibilities

Caregivers at Charis Hearts provide non-medical in-home and/or community care based on the Client Service Plan. Responsibilities may include:

• Client Support & Care
  Perform client wellness checks to ensure their well-being.
  • Assist clients with completing admission documents as needed.
