Charis Hearts, Inc. is a statewide provider of homecare services delivering high-quality, client-centered care through the TennCare CHOICES program.

We offer flexible scheduling, top pay with weekly direct deposit, health benefits, and comprehensive onboarding to support caregivers. Caregivers will provide non-medical in-home and community care based on each client’s service plan, performing wellness checks, assisting with admissions, and supporting clients with ADLs while maintaining strict

Qualifications

• Must be at least 18 years old.
• Must have valid State ID/ Drivers License.
• Strong communication skills – both oral and written.
• Reliable transportation to and from client locations.
• Compassionate and caring nature with a genuine desire to help others.
• Honesty and integrity in all interactions.
• Willingness to attend training sessions provided by referring agencies and/or the Tennessee Department of Mental Health & Developmental Disabilities (MHDD).

Responsibilities

• Caregivers provide non-medical in-home and/or community care based on the Client Service Plan.
• Perform client wellness checks to ensure well-being.
• Assist clients with completing admission documents as needed.
• Help train other caregivers on ADLs in the client's home.
• Provide non-medical support to agency service recipients.
• Be familiar with and follow all rules, policies, and procedures related to service delivery.
• Submit all required documentation accurately and on time.
• Adhere to the individual service plan for each recipient.
• Maintain confidentiality of all client records and information unless authorized by written consent.
• Immediately report any incidents of injury to your supervisor.
• Report suspected abuse or neglect to the appropriate authorities and your supervisor.

Caregiver Responsibilities

Caregivers at Charis Hearts provide non-medical in-home and/or community care based on the Client Service Plan. Responsibilities may include:

• Client Support & Care
  Perform client wellness checks to ensure their well-being.
  • Assist clients with completing admission documents as needed.
  • Help train other caregivers on Activities of Daily Living (ADLs) in the client's home.
  • Provide non-medical support to agency service recipients.
• Compliance & Documentation
  Be familiar with and follow all rules, policies, and procedures related to service delivery.
  • Submit all required documentation accurately and on time.
  • Adhere to the individual service plan for each recipient.
  • Maintain strict confidentiality of all client records and information unless authorized by written consent.
• Safety & Reporting
  Immediately report any incidents of injury involving the service recipient or yourself to your supervisor.
  • Report any suspected abuse or neglect to the appropriate authorities and your supervisor.
