SUMMARY
The Direct Support Professional, Residential, provides support to 4-6 individuals in residential settings or up to 10 in periodic programs, assisting with daily activities, health, medication, and behavioral development. The role involves supporting personal care, housekeeping, recreation, transportation, community integration, shopping, and other daily living activities.
This position can be full-time or part-time, working at the program location and possibly accompanying individuals into the community.

Services and Supports
- Direct Support: Assist with socialization, behavioral development, personal care, housekeeping, recreation, transportation, community orientation, shopping, financial management, citizenship, and daily activities.
- Records: Document progress and activities accurately; review records; maintain confidentiality; complete billing documentation as applicable.
- Management of Individuals’ Assets: Assist with money management and ensure safety of funds and property.
- Relationships: Maintain professional relationships with individuals, families, guardians, and case managers; greet visitors; adhere to Sevita Customer Service Standards.
- Reporting: Report abuse, neglect, medical, behavioral, and other incidents following policies and regulations.
- Rights: Respect and uphold individuals' rights, maintain confidentiality, and practice universal precautions.
Health Care
- Appointments: Accompany individuals to medical appointments; relay information to providers.
- Medications: Administer and document medications accurately; report errors; secure medications properly.
- General Health Care: Monitor health, document concerns, communicate with healthcare staff, and follow directives.
- Medical Supplies and Equipment: Use equipment correctly; report malfunctions.
- Dietary Planning: Support meal preparation, monitor mealtime, and implement dietary plans.
Employment Responsibilities
- Training: Attend orientation and ongoing training; participate in meetings.
- Employee Scheduling: Follow schedule, arrive on time, stay until replacement arrives, notify in case of absence, and fill shifts as needed.
- Teamwork: Collaborate respectfully, communicate openly, and value contributions.
- Workplace Safety: Follow safety policies, report hazards, and participate in safety training.
Maintenance
- Vehicles: Transport individuals safely, follow laws, use safety equipment, and report incidents.
- Housekeeping: Maintain a clean and safe environment, report hazards.
- Maintenance and Repair: Perform minor repairs, yard work, and report issues.
- Safety: Check water temperature, participate in safety drills, and protect individuals during emergencies.
Other Duties

Supervisory Responsibilities
Minimum Knowledge and Skills
The following are required to perform the job:
- High school diploma or equivalent preferred.
- Six months of human services experience preferred.
- Must be 18 years or older.
Certificates and Licenses
- Valid driver’s license in good standing.
- Vehicle registration and insurance if providing transportation.
- Complete all mandatory training and certifications on time.
Physical Requirements
- Heavy work: exerting up to 100 pounds occasionally, 50 pounds frequently, and 20 pounds constantly.
Additional for Tennessee
- Maintain valid driver’s license, meet insurance requirements, and be approved to drive.
Additional Requirements for New Jersey
- Cooperate with inspections and investigations.
- Valid driver’s license in NJ or the state of residence.
- High school diploma or equivalent.
- Submit to drug testing and background checks.
- Complete required training and certifications.
- Be at least 18 years old.
