Responsibilities

• Monitor and report changes in clients’ physical and emotional well-being to the care team.
• Develop and maintain positive relationships with clients, families, and other caregivers.
• Collaborate with other care professionals to develop and implement personalized care plans.
• Maintain accurate records and documentation of client care activities and progress.
