Job summary

Adapt Forward in Modesto, CA is seeking compassionate caregivers to provide non-medical in-home support for seniors and adults needing assistance. Responsibilities include ADLs, safety monitoring, meal prep, light housekeeping, transportation, companionship, and coordination with families.

Qualifications

• Active HCA Registration through CDSS with fingerprinting/background check.
• Valid driver’s license.
• Current TB test and physical within the last 90 days.
• Prior caregiving experience preferred but not required.
• Friendly, patient, adaptable, and professional demeanor.
• Strong communication skills and willingness to coordinate with families and the Amada care team.

Responsibilities

• Assist with ADLs including bathing, grooming, dressing, toileting, mobility, and transfers.
• Provide medication reminders.
• Prepare meals and ensure hydration.
• Perform light housekeeping and laundry.
• Provide transportation using caregiver's vehicle with mileage reimbursement.
• Offer companionship and social engagement.
• Monitor safety and prevent falls.
• Communicate updates to family as appropriate.
• Support memory care needs with Alzheimer’s and dementia-friendly approaches.
• Collaborate with home health, hospice, rehab, and other providers for continuity of care.

• Assistance with Activities of Daily Living (ADLs): - Bathing, grooming, hygiene - Dressing and toileting support - Mobility assistance and safe transfers - Incontinence care
• Medication reminders
• Meal preparation and hydration support
• Light housekeeping and laundry
• Transportation using caregiver’s vehicle (with mileage reimbursement)
• Companionship, social engagement, routine support
• Safety monitoring and fall prevention
• Communication updates to family as appropriate
• Support for memory care needs (Alzheimer’s & dementia-friendly approaches)
• Collaboration with home health, hospice, rehab, and other providers for continuity of care (non-medical role)
• CAREGIVER CERTIFICATION & REQUIREMENTS:
• Active HCA Registration through CDSS (includes fingerprinting/background check)
• Valid driver’s license
• Valid auto insurance for transportation assignments
• Current TB test and physical within the last 90 days
• Prior caregiving experience preferred but not required
• Friendly, patient, adaptable, and professional demeanor
