CareLinx by Sharecare is seeking experienced in-home caregivers in Rockford, IL to provide non-clinical support to health plan members and private clients. Work is on-site at client homes with flexible part-time and per-diem schedules; you manage your availability through the CareLinx app.

As a contractor, you'll join a dedicated CarePartner team to find suitable matches based on location and your availability.

Formación

• Minimum 6 months caregiving experience or relevant certification.
• Access to a smartphone for scheduling, documentation, and timesheets.
• Ability to communicate in English; bilingual a plus.

Responsabilidades

• Assist clients with activities of daily living as assigned.
• Document care tasks in CareLinx app during each shift.
• Report changes to client condition to the CareLinx clinical team.
• Submit timesheets on time through the app.
• Communicate case updates to the CarePartner team.
• Protect client privacy and safety at all times.

Once onboarded,you'llwork alongsidethededicatedCarePartnerteam focused on finding you the right matches — based on location, client needs, and your availability. WHAT YOU'LL DO

Core responsibilities for all caregivers:

Assistclients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping

Complete and document all care tasks accurately in theCareLinxmobile appduringeach shift

Monitor and report changes in a client’s condition or self-care abilities to theCareLinxclinical team

Submit timesheets on time through theCareLinxapp

Communicate with your CarePartnerregardingany case updates or client changes

Ensure patient privacy, dignity, and well-being in a safe and secure environment

Maintain client confidentiality at all times

Additionalshift opportunities (based on client needs):

• Transportation and errand support
• Hospital discharge escort — safelyaccompanyingmembers home after discharge
• Telehealth navigation — helping members connect with virtual doctor visits

WHAT WE'RE LOOKING FOR

Required qualifications:

• Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)
• Accessto a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)
• Ability to communicate in English (bilingual caregivers strongly encouraged to apply)
• Reliable transportation to and from client homes
• Must pass a background check

Program-specific requirements (may apply depending on assignment):

• Completion of program-specific training prior to accepting shifts
• Medical clearance such as TB test, physical exam, or drug screening
• Valid driver's license and MVR check if accepting driving shifts
• Additionaldocumentation may berequiredbased on state law or client contract
