What You Will Do:

Implement customized care plans that nurture the whole person in mind, body, and spirit, treating clients like family and enhancing their quality of life.

Essential Functions:

• Deliver exceptional care in clients' homes.
• Execute personalized weekly schedules and care plans.
• Perform daily tasks safely and professionally (ADLs).
• Provide companionship and social/emotional support.
• Assist with personal care activities, mobility, light housekeeping, and meal prep.
• Maintain safety, order, and client enjoyment.
• Provide transportation to appointments as needed.
• Document tasks and report changes in clients' conditions.
