Job summary

HomeCentris Healthcare, LLC is seeking a compassionate caregiver to provide in-home support, personal care, and companionship. You will assist with ADLs, medication reminders, meals, light housekeeping, and transportation while documenting care and communicating with clients and families in English.

The ideal candidate has excellent communication, a caring attitude, reliable transportation, and CPR certification. Prior home health experience is preferred, and background checks are required.

Qualifications

• High school diploma or equivalent.
• Previous caregiver or similar experience preferred.
• Fluent in English with excellent verbal and written communication skills.
• Active CPR Certification.
• Ability to pass background checks and state regulations.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) including bathing, grooming, dressing, toileting, and mobility.
• Provide medication reminders and assist with medication management per care plans.
• Prepare and serve nutritious meals according to dietary needs.
• Perform light housekeeping to maintain a clean and safe home environment.
• Provide companionship and engage clients in activities and conversation.
• Communicate care needs and changes clearly with clients and families in English.
• Escort clients to appointments and errands using safe transportation.
• Maintain accurate daily activity logs and observations of health or behavior.

Position Overview:

We are currently seeking a compassionate and dedicated Caregiver to join our team. The Caregiver will be responsible for providing assistance with daily activities, personal care, and companionship to our clients in their homes. The ideal candidate will have excellent communication skills, along with a genuine passion for helping others.

Responsibilities:

• Assistance with Activities of Daily Living (ADLs): Assist clients with personal care tasks, including bathing, grooming, dressing, toileting, and mobility.
• Medication Reminders: Provide medication reminders and assist with medication management as needed, following client-specific care plans and instructions.
• Meal Preparation: Prepare and serve nutritious meals and snacks according to clients' dietary preferences and restrictions, ensuring food safety and sanitation standards are met.
• Housekeeping: Perform light housekeeping tasks, such as laundry, sweeping, mopping, and tidying up living spaces, to maintain a clean and safe environment for clients.
• Companionship: Provide companionship and emotional support to clients, engaging them in conversation, recreational activities, and cultural interests to promote socialization and mental stimulation.
• Communication: Communicate effectively with clients and their families in English to ensure clear understanding of care needs, preferences, and instructions.
• Transportation: Escort clients to appointments, errands, and recreational activities as needed, using reliable transportation and adhering to safety protocols.
• Documentation: Maintain accurate and up-to-date records of care provided, including daily activity logs, medication administration records, and any changes or observations in clients' health or behavior.

Qualifications:

• High school diploma or equivalent.
• Previous experience as a caregiver, home health aide, or similar role preferred.
• Fluent in English with excellent verbal and written communication skills i
• Compassionate, patient, and empathetic attitude towards clients.
• Reliable transportation and a valid driver's license (preferred).
• Ability to pass background checks and physical as required by state regulations.
• Active CPR Certification
