Job summary

Meadow Falls of Middletown is seeking a compassionate Caregiver (PRN) to join our team. You will provide high-quality care and companionship to residents, supporting physical, emotional, and social well-being.

Responsibilities include following individual care plans, assisting with daily living activities, and helping residents stay engaged in community activities. High School diploma or equivalent required; experience preferred; flexible scheduling offered.

Responsibilities:

• Following the resident's individual care and service plan in providing daily wellness checks, housekeeping, incontinence management, and assistance with activities of daily living such as bathing, dressing, eating, toileting, hygiene, and mobility.

• Escorting residents to activities, meals, and outside appointments as needed.

• Assisting in development and updating of each resident's individual service plan and charting the daily services you are providing.

• Encouraging socialization and participation in community activities.
