Job summary

Rochester Regional Health is seeking a Caregiver (Certified Home Health Aide) for ElderONE per diem shifts in the Rochester area. You will assist participants with daily living activities, personal care, meal preparation, and light housekeeping in home settings.

You will communicate patient status to nurses and supervisors, document care accurately, and follow HIPAA and department protocols while supporting a flexible per diem schedule.

Qualifications

• HHA certification or eligibility with CNA + written test and 1 day HHA skills class within 30 days of hire.
• Nursing assistant training and competency evaluation from an out-of-state program accepted.
• For HHA lapse within 2 years, complete required skills class and tests within 30 days.

Responsibilities

• Provide daily living assistance per care plan: personal care, light housekeeping, meals, and errands.
• Maintain clear communication of patient health status with family, nurses, and supervisors.
• Accurately document care on activity sheets and ensure HIPAA compliance.
• Adhere to HIPAA and organizational protocols and regulations.

Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errandsCommunication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularlyDocumentation. Accurately and completely document client care on activity sheetCompliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirementsREQUIRED QUALIFICATIONS:HHA Certified or Ability to meet one of the following eligibility requirements for Competency:Currently certified CNA along with completion of written test and one day HHA Skills competency class within 30 days of hire.Documented home health aide or nurse aide training and competency evaluation from an out-of-state training programThe Certified HHA who wishes to be employed after a two-year lapse in employment as a HHA must take the 1 Day Skills Competency Class. They must complete a written test and pass the one day HHA Skills competency class within 30 days of hire and before working as an HHA. If the candidate has worked as a Certified HHA within the past 24 months, and is actively certified, no class is needed.Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 30 days of hire.Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 30 days of hire.PREFERRED QUALIFICATIONS:High school diploma or equivalentFor ElderONE employment and other areas of elderly care: 1 year experience working with the frail elderly populationCompassionate, patient focused and dependable professionalCritical thinking skills, decisive judgment and the ability to work with minimal supervisionExceptional documentation and communication skillsAbility to follow verbal and written instructionsEDUCATION:LICENSES / CERTIFICATIONS:PHYSICAL REQUIREMENTS:Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. It may be modified in the future and eligible for additional pay components.
