Job summary

Mosaic Gardens at Chandler is seeking a compassionate Caregiver to provide hands-on resident care including bathing, grooming, dressing, toileting, mobility, meals, hydration, and comfort.

You will monitor health, document findings, communicate with the Caregiver Coordinator, and support engagement activities within a supportive, team-oriented assisted living environment; some evenings and weekends may be required.

Qualifications

• Caregiver certification required
• High school diploma or equivalent required
• Previous long-term care experience preferred
• Ability to read, write, and communicate effectively in English
• Must demonstrate patience, empathy and a genuine desire to support seniors
• Knowledge of assisted living, memory care and related regulations preferred
• Ability to work collaboratively in a fast-paced, team-oriented environment
• Flexibility to work evenings, weekends and holidays as needed

Responsibilities

• Provide compassionate, hands-on care to residents (bathing, grooming, dressing, toileting, mobility)
• Assist residents with meals, snacks, hydration, and feeding as needed
• Support ambulation, positioning, and range-of-motion activities
• Monitor residents' comfort and address needs promptly
• Monitor and record vital signs, weight, and intake/output
• Report changes to Caregiver Coordinator or Executive Director
• Document care and observations per Mosaic policies and regulatory requirements
• Support resident engagement and activities

• Report any changes in residents’ physical, emotional, or cognitive status to the Caregiver Coordinator or Executive Director.
• Document all care, observations, and resident interactions in compliance with Mosaic policies and regulatory requirements.
