Workforce Connections, Inc. (WCI) is seeking a Career Planner to provide person-centered case management and employment planning services to help individuals and families achieve employment and economic stability.

You will assess strengths and barriers, develop individualized employment plans, coordinate supportive services, and connect participants with education, training, and community resources while partnering with employers and community organizations.

Formación

• Bachelor's degree in human services, education, business, or a related field preferred.
• Experience providing case management or employment services is preferred.
• Strong communication, organization, and time management skills required.
• Ability to build relationships with participants, employers, and partners.

Responsabilidades

• Provide participant intake, eligibility determination, case management, and employment planning services.
• Develop individualized employment and service plans that support participant success and long-term self-sufficiency.
• Connect participants with employment opportunities, education, training, supportive services, and community resources.
• Build partnerships with employers, educational institutions, and community organizations to expand opportunities for participants.
• Maintain accurate documentation and ensure compliance with program requirements and organizational policies.
• Manage an assigned caseload while monitoring participant progress and program outcomes.
• Perform other duties consistent with the responsibilities of the position.
