Job summary

Nottingham City Council's Social Care Reablement team provides in-home support to citizens recently discharged from hospital or other services. The role involves personal care, care plan assessment, and promoting independence and wellbeing within a flexible rota, including weekends. DBS enhanced checks are required for this post.

Qualifications

• Experience undertaking personal care tasks to a high standard with respect and care.
• Ability to assess social care and health needs and develop/update Person Centred Care Plans.
• Knowledge of rights, needs and feelings of vulnerable adults from diverse backgrounds.
• Ability to administer medication in line with policies.

Responsibilities

• Provide personal care in citizens' homes as directed by care plans.
• Assess needs and develop/maintain person-centred care plans.
• Promote independence and wellbeing, supporting social engagement.
• Work flexible hours including weekends as rota dictates.
