Job summary

Episcopal Health Services Inc. is seeking a Discharge Coordinator to support discharge planning and patient throughput under the direction of case managers and social workers. The role requires coordinating transfers, referrals, approvals and documentation with care facilities, providers and families.

The position is full-time (75 hours biweekly) with day shifts from 11:00 AM to 7:30 PM, located at St. John’s Episcopal Hospital in the Rockaways region.

Qualifications

• Bachelor’s Degree in Health care or related field.
• Two years of work-related experience in healthcare.
• Understanding of Medical terminology and processes.
• Proficiency in Microsoft Office Suite.
• Highly organized analytical thinker with good problem-solving skills.
• Excellent interpersonal skills and appropriate telephone etiquette.

Responsibilities

• Coordinate discharge-related tasks with the discharge planners, case managers and social workers.
• Communicate with facilities and providers to arrange transfers and bed availability.
• Follow up on referrals and obtain necessary authorizations.
• Manage documentation and information flow to care teams and families.
• Maintain updated lists of resources and transfer options.
• Generate monthly reports on discharge planning processes and outcomes.
