Job summary

Bridgeport Hospital, part of Yale NewHaven Health, is seeking a Clinical Pharmacist II to join the Pharmacy team in Bridgeport, CT. The role supports medication management, safety, and patient-centered care within a high reliability organization.

Requirements include a B.S. or PharmD from an accredited college; two years' experience preferred; ASHP PGY1 residency preferred but not required; CT pharmacist license or eligibility. Strong communication and collaboration are essential.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is preferred, but not required.
• Minimum of two years of relevant experience specific to practice area is required.
• Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut.

Responsibilities

• Customer Service Quality: Demonstrate and promote CHAMP behaviors.
• Medication Management: Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs.
• Medication Preparation and Distribution: Demonstrate competency in pharmacy automation and technology as applicable to work area.
• Medication Safety, Regulatory Affairs, and Quality Assurance: Adhere to all departmental policies, procedures, protocols, and guidelines.
• Leadership: Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team members, via educational inservice or as a preceptor.
• Financial/Volume Results: Contribute to departmental cost-reduction/revenue generation.
