Job summary

Baptist Health in New Albany, Indiana, is seeking transitions of care pharmacists to lead target interdisciplinary programs and clinical services, ensuring pharmaceutical care is integrated throughout the BH system.

In this role, you will participate in all aspects of the medication-use system, provide patient-centered pharmaceutical care in your assigned areas, and serve as a clinical resource and liaison to other departments and external groups.

Qualifications

• Bachelor’s degree from accredited College of Pharmacy.
• PGY-1 Residency preferred.
• A minimum of one year pharmacist experience required.
• Indiana and Kentucky Pharmacist License required.

Responsibilities

• Transitions of care pharmacists lead interdisciplinary programs and clinical services within the Baptist Health system.
• Participate in all necessary aspects of the medication-use system and provide patient-centered pharmaceutical care.
• Serve as clinical resources and liaisons to other departments, hospital personnel, or external groups.
• Conduct practice advancement projects and quality/regulatory initiatives to improve medication-use processes.
