Schedule Details/Additional Information: This is a remote work from home role. High speed internet is required. Hours of operation are 8am to 8:30pm. Occasional holiday coverage. Must live within 1 hour of Mint Hill office.

Pay Range: $37.50 - $56.25

Responsibilities
  • Facilitates a lifelong, proactive partnership with patients to enhance and personalize management of health-related needs.
  • Assess needs, plan, coordinate, and evaluate services to empower individuals and families to access resources and adopt healthy lifestyles.
  • Focus on population management, patient self-management support, transitions in care, resource connection, and appropriate resource utilization.
  • Collaborate with the multidisciplinary care team (providers, pharmacists, social workers, behavioral health specialists, and nurses) to achieve timely, appropriate patient management.
  • Utilize RN process to guide clinical outcomes, resource utilization, and improvement strategies.
  • Document plans of care and maintain accurate case records across care settings.
  • Maintain knowledge of reimbursement criteria and documentation for Medicaid, Medicare, and Managed Care.
Qualifications
  • BSN required. Masters preferred.
  • Current RN license or temporary license as a Registered Nurse in the state of work/residence; or National License Compact (NLC) state primary residency with licensure requirements met in home state; or for non-NLC states, current RN license or temporary license as a Registered Nurse Practitioner in the state of work.
  • Two years of healthcare experience; experience in case management/discharge planning in acute care, home care, LTC, physician office, or managed care setting.
  • Professional certification required within 3 years of hire per departmental protocol.
  • Clinical competence in disease management and case management principles.
  • Excellent interpersonal, negotiation, problem-solving, organizational, and time management skills; ability to work independently and as part of a care team.
  • BLS required per policy guidelines.
Physical Requirements

Walking, standing, lifting up to 35 pounds, reaching, bending, stooping. Ability to travel between locations. Requires frequent verbal and written communication in English. Visual and auditory requirements met. Ability to respond quickly to changes in assignments. Occasional exposure to dust, fumes, and chemicals.
