A prestigious Healthcare company in The Bronx, NY is seeking a Care Transitions Nurse Manager (RN) to oversee complex nursing cases and facilitate interdisciplinary coordination. This role offers a salary range of $135,000 - $140,000 annually, benefits including medical, dental, and vision, and generous PTO. Ideal candidates will have a valid NY RN license and substantial experience in home health care.

Qualifications

• 3+ years' experience as a Registered Nurse / RN.
• 2+ years of recent experience at a Certified home health agency (CHHA).
• 1+ year of recent supervisory experience.

Responsibilities

• Make field visits but will not carry their own caseload.
• Assist and oversee complex cases as a SME.
• Communicate with interdisciplinary teams to coordinate care.
