As a Care Transition Navigator, you will:

• Cultivate and nurture relationships with referral sources, patients, and families.
• Collaborate closely with facility discharge planners to ensure timely and effective discharges.
• Interact directly with patients and families to address post-transition needs for a safe and effective transition home.
• Facilitate communication among care team members to enhance the transition process.
• Deliver exemplary care along the care continuum.
• Collaborate with operational leaders to support team culture, address challenges, promote accountability, and drive continuous improvement.
• Utilize tools and market knowledge to understand market dynamics and identify growth opportunities.
• Consistently meet monthly individual performance goals.
