Responsibilities

• Coordinate post-discharge care and schedule PCP follow-up within 7 days.
• Reconcile medications and manage Transition of Care Documentation in EHR.
• Collaborate with hospitals, IPAs and health plans for data sharing.
• Develop dashboards and report transition care activities monthly.
