Job summary

Kaweah Health is seeking a Care Coordination Specialist in Visalia, CA to collaborate with RN Case Management and insurance companies. The role centers on gathering clinical data, supporting timely discharge planning, and coordinating with multidisciplinary teams to optimize patient outcomes.

The position requires education in health-related field, utilization review experience, and strong communication; proficiency in Excel is preferred for documentation and data management.

Qualifications

• Two years of college or equivalent training/experience in health-related field.
• Utilization review and/or coding experience.
• Must understand basic medical terminology.
• Strong organizational and multitasking skills, with the ability to manage multiple patients and tasks simultaneously.
• Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare professionals.
• Knowledge of discharge medication delivery processes and procedures, including coordination with the outpatient pharmacy and the patient and or family.
• Ability to work collaboratively with interdisciplinary team members and healthcare professionals.
• Strong computer skills, with proficiency in electronic medical record documentation preferred. Proficient in using Excel for documentation.

Responsibilities

• Reviews utilization review data for assigned caseload to ensure inpatient records are current.
• Refer variances to Case Management with appropriate documentation.
• Review admission and continued stay using established criteria.
• Coordinate transfers to long-term care facilities when indicated.
• Assist with discharge planning services including equipment, home care, and transportation coordination.
• Participate in Continuum of Care rounds and collaborate with case management teams.
