Responsibilities
The Care Transition Coordinator’s primary responsibility is to facilitate a seamless transition for patients discharging from a facility setting to the care of an LHC Group hospice agency.
Responsible for achieving admission goals as established at hire or during annual review.
Assists the agency with preparing for and accepting care of the patient post-discharge.
Assists the Administrator with execution of contracts for facility-based services for hospice patients.
Explains hospice services and procedures to patients and families.
Involves family and caregivers in education and assesses post-discharge educational needs.
Participates in bi-weekly IDG meetings to update on accounts, needs, and progress.
Education and Experience

• One year hospice or hospital case management experience.
• Current RN, LPN, or SW licensure in the state of practice.
• Excellent organizational and communication skills.
• Thorough understanding of hospice criteria and coverage guidelines.
• Proficient computer skills.
• Current CPR, valid driver’s license, insurance, access to a vehicle or public transportation.
