What You’ll Do:

As a Care Team Support Specialist, you'll be the operational backbone for our clinical team — ensuring patients are scheduled, prepared, and supported across every touchpoint. Whether you're prepping charts, coordinating with providers, or checking in with patients after a visit, your work enables our pharmacists to deliver high-impact, compassionate care.

Core Responsibilities

  • Schedule patient visits and coordinate follow-ups with clinical pharmacists
  • Conduct pre-visit calls to gather relevant medication and health history
  • Prepare charts with key data for pharmacist review
  • Perform outreach to patients post-visit for follow-up and education
  • Support medication reconciliation by collecting accurate medication info
  • Collaborate with physician offices, care coordinators, and caregivers to ensure continuity
  • Monitor and escalate medication risks or care gaps to clinical leads
  • Navigate multiple systems (EHR, communication tools, task dashboards) efficiently
  • Participate in daily team huddles and contribute to continuous improvement

What We’re Looking For

Required

  • CPhT (Florida) or Certified Medical Assistant (CMA/RMA)
  • 2+ years in a patient-facing healthcare role (clinic, retail pharmacy, telehealth, etc.)
  • Strong communication skills and comfort speaking with older adults
  • Detail-oriented, organized, and tech-savvy (Google Workspace, EMRs, Slack)
  • Reliable internet and professional home work setup
  • Comfortable working in a fast-paced, mission-driven virtual team

Preferred

  • Experience in Athena
  • Experience in virtual care, pharmacy quality programs, or Medicare Advantage
  • Familiarity with medication reconciliation, HEDIS measures, or risk assessments
  • Be part of core clinical team — with a real voice
  • Deliver care that puts patients and clinicians first
  • Work with a mission-driven, collaborative, and scrappy team
