Job summary

Community Health Centers is seeking a licensed LPN to support patient care coordination and clinical operations at our Florida centers. You will assist with immunizations, medication administration, and lab coordination while ensuring accurate documentation and patient flow.

You will collaborate with physicians and care teams, manage patient schedules, and participate in quality improvement activities. Travel to CHC locations may be required, with a focus on safe, compassionate care.

Qualifications

• High school diploma or equivalent and completion of accredited LPN program.
• Florida LPN license and current BLS certification.
• Minimum 2 years’ experience as LPN in healthcare/clinic setting preferred.
• Experience with electronic medical records and health IT.

Responsibilities

• Coordinates and facilitates structured communication to center care team members and providers related to patient care coordination.
• Participates in team meetings to communicate patient care and operational requirements.
• May lead huddles emphasizing a team-based approach to care.
• Administers immunizations and medications as ordered while ensuring safe medication administration.
