Job summary

Centerstone is a leading nonprofit behavioral health system seeking a part-time professional to assist clients in developing life skills and goals. You will coordinate with a multidisciplinary team to ensure accurate clinical records, offer crisis intervention, and connect clients with community resources and schools.

Ideal candidates have an Associate's degree and two years of direct youth and family experience; flexible, compassionate, and customer-focused service is essential.

Qualifications

• Associate's degree required.
• Two years of experience or two years of college coursework in related field may be substituted for degree.
• Two years professional experience working directly with youth and families in relevant settings preferred.

Responsibilities

• Coordinates client care with a multidisciplinary team to develop and maintain clinical records.
• Provides crisis intervention and prevention to clients and families.
• Teaches activities of daily living and provides services per guidelines.
• Connects clients with community resources and maintains communications with schools and family service providers.
• Delivers crisis intervention both on-site and via telephone.

Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their familes.ESSENTIAL DUTIES & RESPONSIBILITIESCoordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, ensuring compliance standards.Teaches and trains clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines and/or protocols.Connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFC’s and other family service providers that the client and their family participates with or is receiving services from.Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.KNOWLEDGE, SKILLS & ABILITIESEffectively communicate via written, verbal, in person and virtual methods.Customer service skills.Time management skills.Analytical skills.Knowledge of community resources.QUALIFICATIONSEducation LevelAssociate's degree required.
