What You Will Do

The Care Specialist primarily works in patients' homes and communities (90%) and virtually (10%) to deliver chronic care management for high-complexity patients. Your role includes informing patients about our services, enrolling them, and coordinating care to promote timely access, prevent hospital readmissions, support self-management, and connect patients to community resources.

Key Responsibilities
  • Conduct outreach via phone, in-person visits, mailings, including cold calls and door knocks.
  • Enroll patients and gather key data during interactions.
  • Meet patients in their homes or community to assess needs and set health goals.
  • Coordinate care with interdisciplinary teams.
  • Support patients in achieving healthcare goals related to quality measures.
  • Analyze patient data to identify needs and treatment goals.
  • Assess physical conditions and determine referrals.
  • Explain medical information to patients clearly.
  • Communicate with primary care providers regarding transitions and referrals.
  • Build rapport using motivational interviewing techniques.
  • Conduct extended in-person appointments and follow-up visits.
  • Accurately document encounters and prepare reports.
  • Participate in team meetings and clinical rounds.
  • Perform other duties as needed.
