Responsibilities
  • Conduct outreach via phone, in-person, and mail to enroll and support patients.
  • Perform needs assessments and develop personalized care plans.
  • Coordinate with interdisciplinary teams and community resources.
  • Support patient self-management and adherence to care plans.
  • Document interactions accurately and timely.
Qualifications
  • Minimum 2 years in community health or related roles.
  • High school diploma or equivalent.
  • Valid driver’s license, reliable transportation, and ability to travel.
  • Experience with chronic care management or caring for chronically ill/elderly patients.
  • Technologically proficient and able to complete training.

Additional Details
