Job summary

Insight Global is seeking an experienced Care Manager or Home Health Nurse for a community-based role in Massachusetts. The primary focus is coordinating care for dual-eligible members with complex medical, behavioral, and social needs, including comprehensive assessments to improve outcomes.

The ideal candidate will hold an MA license (RN, LCSW/LICSW, or LMHC) and possess an ADN or MSW/MHC degree, with 3+ years of RN/Behavioral Health experience or 1 year plus 2 years with complex populations.

Qualifications

• Must hold a Massachusetts license (RN, LCSW/LICSW, or LMHC).
• Degrees accepted: ADN or MSW/MHC (as applicable to license).
• Experience in behavioral health or care management preferred.

Job description

Pay Rate: $40-$53/hr

Schedule: Monday-Friday | 8:00 AM - 5:00 PM

Must-Haves

• Current Massachusetts license as one of the following:
• Registered Nurse (RN)
• Licensed Clinical Social Worker (LCSW/LICSW)
• Licensed Mental Health Counselor (LMHC)
• Associate's Degree in Nursing or Master's Degree in Social Work/Mental Health Counseling
• Either:
• 3+ years of RN/Behavioral Health Clinician experience, or
• 1 year of RN/Behavioral Health experience plus 2 years working with complex populations
• Valid driver's license, reliable transportation, and auto insurance

Ideal Candidate: Experienced Care Manager or Home Health Nurse with strong care coordination experience.

Preferred Experience

• LTSS and Dual-Eligible populations
• Population health or community health
• Behavioral health
• Case Management Certification (CCM) preferred
