Care Manager – EverCare Mobile Health

Location: Multiple - Treasure Valley, Magic Valley, Eastern Idaho, Payette, Ontario, Central Oregon

Schedule: Full Time

Compensation: $20 - $28 per hour

L1: Care Partner is new to the industry or new to Community Health Integration (CHI).

Wage range: $20 - $22

L2: Individuals have been with the organization for one year. Or has a CPSS Certification.

Wage range: $22 - $24

L3: Individual must have a Bachelor degree.

Wage range: $24 - 26

L4: Care Manager - Must have a BA or BS in Social Work or Psychology

Wage range: $26 - $28

$18- $28 per billable hour

$12.50 per non-billable hour

Key Responsibilities

Assist with Community Health Integration and navigation for long term care residents.

Conduct assessments, treatment plan reviews, and progress notes.

Maintain accurate and timely documentation in compliance with payer and company standards.

Participate in supervision and team meetings.

Collaborate with other treatment providers to ensure comprehensive care.

Adhere to HIPAA privacy and compliance regulations.

Assess emergency situations and escalate concerns as necessary.

Provide consistent support to clients, promoting personal growth and accountability.

Qualifications

CPR certification (preferred).

Ability to pass an enhanced IDHW/ORHW background check.

Valid driver’s license, reliable transportation, and proof of insurance.

Experience using electronic health records (preferred).

Comfortable working in a community-based setting.
