Job summary

BayCare is seeking a Care Partner-LPN to support patient care coordination within a large Tampa Bay network. You will work under supervision of RN Care Partner Coordinators to contact patients and providers by phone and help enhance the quality of care and patient experience.

The position involves reviewing reports, reaching out to patients, and occasional travel within the service area. A Florida LPN license and valid driver’s license are required, with a hybrid location and day shift schedule.

Qualifications

• Required LPN licensure (Florida)
• Required Driver’s License – State of Florida
• Minimum 1 year as LPN
• Minimum 3 years in healthcare
• Preferred 2 years in Family Practice
• Preferred 1 year with Health Insurance
• Preferred 2 years in a Medical Office

Responsibilities

• Assist with contacting patients and providers telephonically to improve health and care quality.
• Review daily Excel reports and reach out to patients and providers as part of BayCare Physician Partners.
• Perform other duties as assigned.
• Potential travel within the BayCare service area using own transportation.

The Care Partner-LPN responsibilities include:

• Reviews daily Excel reports, reaching out to patients and providers and supporting the efforts of BayCare Physician Partners.

• Potential travel within the BayCare service area using own transportation.

• Schedule: Days will vary. Day shift.

Certifications and Licensures:

• Required LPN (Licensed Practical Nurse) License – State of Florida
• Required Driver’s License – State of Florida
