Job summary

Milestone is seeking compassionate Care Partners to support residents with daily living needs in our Assisted Living and Memory Care communities. You’ll provide personal care, assist with meals, and communicate changes in residents’ conditions to the Health & Wellness team.

This role emphasizes safety, high-quality individualized care, teamwork, and adherence to policies. Hours include weekends and rotating holidays; shifts may be day, evening, or night. On-site training provided.

Qualifications

• High School Diploma or equivalent.
• Familiarity with Microsoft Word and Excel.
• Strong interpersonal, verbal, and written communication skills.
• Ability to communicate clearly in English and complete documentation accurately.
• Strong organizational skills, reliability, and attention to detail.
• Ability to meet health requirements including TB testing.
• Ability to pass fingerprinting, background checks, and screenings.
• Certifications as required by state.

Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner.
• Document changes in resident condition per company policy and state requirements.
• Support Continuous Quality Improvement by identifying issues and communicating them to leadership.
• Adhere to all company policies and regulations including HIPAA, OSHA, and infection control guidelines.
• Promote a safe working environment and report unsafe actions immediately.
• Report any suspected elder maltreatment or abuse according to policy.
• Communicate effectively with fellow Care Partners regarding resident needs.
• Assist with meal service including serving, clearing, and dining room cleanliness.
• Foster teamwork, respect, and positive interactions with residents, families, and coworkers.
• Perform other duties as assigned.

About The Role
