Job summary

Child & Family Services Inc in Fall River, MA seeks a Care Navigator to connect clients with behavioral health services, including same-day or next-day access. Full-time schedule with varied hours and local travel to provide services in multiple settings.

Responsibilities include pre-screening, intake coordination, referrals, care transitions, crisis intervention, and coordination across health and behavioral health providers.

Qualifications

  • Bachelor’s Degree level degree in Social Work, Sociology, Psychology or a related human services field.
  • Must have at least one year of experience working in mental health or substance use services.
  • Demonstrates cultural humility, with an understanding of ethnic, cultural, and/or linguistic considerations for populations served.
  • Bilingual individuals preferred to best support the needs of our clients and families within the community.

Responsibilities

  • Facilitate walk-in process with clients
  • Complete pre-screening assessment with client
  • Identify client needs and determine service options with client
  • Schedule client intake and follow up appointments as appropriate
  • Make clinic referral and wait list phone calls
  • Make internal and external referrals as appropriate
  • Support transitions of care
  • Engage in care coordination across health and behavioral health services
  • Provide crisis intervention strategies
  • Maintain record-keeping as required by agency and funding sources
  • Provide clients with consistent, high-quality care through evidence-based, culturally competent, person- and family-centered treatment

Job description

The Care Navigator provides behavioral health services, including access to same-day or next-day services. The Care Navigator acts as a liaison to connect clients and families with the appropriate services. Services may include outpatient therapy, recovery support services, and school-based services.Full-time Schedule: Mon- 8a-5p, Tues-8a-3:30p, Wed- 11:30-8p, Thurs- 8a-4:30p, Fri-8a-5:00pChild and Family Services recognizes the power in Diversity and believes that Equity is a Human Right.$1 per hour Language Differential!
