Position: Chronic Care Management Specialist (Care Navigator). Location: Morganton, NC – onsite in an outpatient care clinic. Schedule: Monday‑Friday, with occasional weekend work to close out month‑end processes.

Responsibilities

• Be a health hero to over 500 patients across North Carolina.
• Build lasting relationships through regular check‑ins and support calls.
• Use state‑of‑the‑art remote monitoring tools to track patients' health in real‑time.
• Collaborate with clinical pharmacists to optimize patient care plans.
• Educate and motivate patients to achieve their health goals.
• Be the friendly voice and listening ear that patients look forward to hearing from.
• Help reduce hospital readmissions, improve medication adherence, and enhance patients' quality of life.
• Empower patients to take control of their health and provide peace of mind to patients and families.

Qualifications
