A healthcare services organization is seeking a Care Navigator in Missouri to coordinate patient care in senior living communities. Responsibilities include managing patient records, assisting medical providers, and establishing relationships with residents and their families. We provide onsite geriatric primary care services in addition to our physical, occupational, and speech therapy offerings. As the Care Navigator, you will assist in the coordination of patient care in an assigned community or communities. You are responsible for providing administrative and clinical support to the medical providers and the integrated services team. This position requires independent decision making with limited oversight.
