Care Navigator, Dementia Specialist (Healthcare) – Jewish Family Service Association of Cleveland (JFSA of Cleveland)

JFSA of Cleveland is celebrating its 150th anniversary! We have been a Top Places to Work winner for the last 14 consecutive years and offer a broad array of social services to residents across the Eastern suburbs of Cleveland, Ohio. We have an immediate opening for a Care Navigator, Dementia Specialist. This role is full-time, salary commensurate with experience, and office-based.

Responsibilities
  • Helps to establish infrastructure (e.g. eligibility requirements) for new dementia care navigation program.
  • Conducts intake, and provided case management, care coordination, counseling, and support to individuals with dementia and their family caregivers.
  • Works with family members to develop plans to improve their loved ones quality of life, avoid crises, and reduce caregiver stress.
  • Assists with the implementation of 'Savvy Caregiver' training program.
  • Tracks data/maintains record keeping as outlined in the HHS Care Paths grant
  • Assesses and identifies the needs of individuals with dementia as well as their family caregivers.
  • Links clients with appropriate resources while guiding family members through making decisions regarding their options.
  • Helps clients follow through by making appointments and arranging transportation.
  • Maintains communication with and between medical, social services, and other providers.
  • Manages crises and meets clients at hospitals or in their homes. Intervenes to bring hospital or other staff up to speed on the client's needs, assists with discharge plans, and provides respite resources to family members.
  • Advocates for clients and assists with applications for government benefits such as Medicaid, housing, and veterans benefits.
  • Helps family members process the reality of their loved ones dementia and work to manage family members expectations.
  • Documents services provided to clients in the client record as well as electronically within agency timelines.
  • Delivers regular reports to monitor program progress and impact.
  • Prepares and/or delivers presentations and conducts workshops for agency committees, staff, or outside organizations as assigned.
  • Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices. Also contribute to the control of hazards in specified area of responsibility.
  • Performs other duties or special projects as required or as assigned.
