Job summary

The Utah Department of Health & Human Services, Division of Correctional Health Services, seeks a Care Navigator (Caseworker II) to join a team delivering evidence-based interventions to stabilize mental health and support functioning in custody.

Location is Salt Lake City, in-office; background check and drug testing required; schedule code B; competitive career service. This role emphasizes accountability, treatment, education, and partnerships to improve outcomes and community safety.

Qualifications

• Licensure as Social Service Worker or Certified Substance Use Disorder Counselor or working toward licensure.
• Experience with substance use disorder and/or mental health treatment.
• Experience teaching educational classes.
• Experience in case management principles.
• Strong speaking, reading, and written communication skills.
• Knowledge of risk assessments and criminogenic risk factors.
• Familiar with medication treatment for opioid use and other SUDs.

Responsibilities

• Provide care navigation within a correctional health setting.
• Support evidence-based interventions to stabilize mental health in custody.
• Promote accountability, treatment, education, and safe environment.
