Job summary

Kettering Health is seeking a Certified Medical Assistant Care Navigator to support the interdisciplinary medical home care team with care navigation and coordination for high-risk patients. The role emphasizes collaboration with Care Managers and the broader team to implement access and utilization improvements and proactive patient management.

Qualified candidates will have a high school diploma, 1 year clinical experience, and CMA/CCMA/NCMA or RMA certification with a MA program completion.

Qualifications

• High School Diploma or equivalent.
• 1-year clinical experience in a medical/physician office setting.
• Successful attainment and retention of CCMA, CMA (AAMA) or NCMA certification or RMA plus completion of a Medical Assisting program.
• Basic Life Support certification required.
• Must be able to collaborate and communicate with Care Managers and interdisciplinary team.
• Must be able to collaborate with Health Coach and interdisciplinary team in planning follow-up care and scheduling urgent appointments and referrals.

Responsibilities

• Assist the interdisciplinary medical home care team in care navigation activities and coordinate with patients assigned by high risk status or complex care needs.
• Support the Care Manager and team with access and utilization management improvements and proactive patient panel management.
• Uphold the system-wide customer service program standards.
• Perform duties for practice or Network commensurate with certifications or licensure as required.
• Manage caseload according to Regional and Network standards.
