SOUTHWEST BEHAVIORAL CARE, INC. is seeking a part-time Care Navigator to connect clients with community-based services and support long-term recovery. The role involves educating clients, families, and staff about the recovery process and assisting with goal-oriented plans.

Qualifications

• Requires an associate degree or bachelor’s degree in Social Work, Psychology or related field.
• Must be able to obtain required state/federal clearances.

The COE uses community based innovative techniques to assist clients with their quest for long-term sobriety and integration back into healthy living.A Care Navigator provides individualized and community-based services that address opiate use. As a Care Navigator, you will connect clients with services that will help them achieve their personal recovery goals.
