Overview

Care Navigator

Looking for a way to make a difference in people’s lives? As a Care Navigator, you will play a critical role in supporting our members to live the best, rest of their lives. This position acts as the primary point of contact for our members, their families, and our partner senior living facilities. You will help members navigate through the ever-changing healthcare landscape and act as an advocate to ensure they receive the highest quality of care. This position is remote and requires visits and travel to facilities on a regular basis.

Responsibilities
  • Member Engagement
    • Main point of contact for Members and families upon Plan enrollment
    • Perform welcome interview and Health Risk Assessment, routine follow up with members/families
    • Assist members with items including, but not limited to referral management, appointment scheduling, DME ordering, and accessing Plan benefits
  • Facility Engagement
    • Liaison between facilities and the Plan to assist with customer service requests such as billing questions, authorization needs, or member transfers
    • Clinical Support
    • Medical records collection to support new member services and transitions of care
Qualifications
  • 1+ year health care experience with direct patient engagement
  • Senior living experience strongly preferred
  • Passion for customer service
  • Strong verbal, written, and computer skills for communication and system management
