  • Remote role. Candidate must reside in the state of Mississippi. Preferred experience in Discharge Planning and/or in a hospital setting. Licensure is not required.

Position Purpose: Develops, assesses, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on services and benefit options available to improve healthcare access and receive appropriate high-quality care through advocacy and care coordination.

  • Evaluates the needs of the member, barriers to care, resources available, and recommends and facilitates the plan for the best outcome.
  • Develops or contributes to the development of a personalized care plan/service plan and works to identify providers, specialists, and community resources needed for care.
  • Provides psychosocial and resource support to members/caregivers, and care managers to access local resources or services such as employment, education, housing, food, participant direction, independent living, justice, foster care.
  • Coordinates between the member, family/caregivers, and care provider team to ensure services are accessible in a timely manner.
  • Monitors progress towards care plan goals and collaborates with healthcare providers for revisions or further evaluation.
  • Maintains all member information and care management activities to ensure compliance with regulations.
  • May perform on-site visits and provide education on procedures, healthcare provider instructions, care options, referrals, and benefits.
  • Other duties as assigned to meet member and business needs.

Education/Experience: Bachelor’s degree and 2–4 years of related experience. Clinical licensure preferred if applicable. Equivalent experience may be considered.

License/Certification: Current state’s clinical license preferred. For Mississippi Magnolia Health plan: Bachelor’s or Master’s in healthcare-related field with 2–4 years of experience; LSW, LMSW preferred.

Pay Range: $22.50 - $38.02 per hour

Centene offers comprehensive benefits including health insurance, 401K, tuition reimbursement, paid time off, holidays, and flexible work options. Actual pay is based on skills, experience, education, and other factors. Total compensation may include incentives.
