Make an Impact as a Care Navigator!

The NewPath Child & Family Solutions Care Navigator is responsible for assisting clients and families in navigating the continuum of care by utilizing assessments, developing treatment plans, and coordinating referrals to maximize effective, individual service provisions.

Location: This position is with our Hamilton County Outpatient Department working through our NewPath Central location at 4721 Reading Rd, Cincinnati, OH 45237.

What You Will Be Doing
  • Educate clients and families on the treatment recommendations and care options based on their presenting problems and personal goals.
  • Assess the client's symptoms, functioning, strengths, problems and needs.
  • Coordinate the necessary evaluations and assessments to identify each youth's needs, DLA-20, ISP, PHQ9, Ohio Scales, CANS etc.
  • Provide transportation as appropriate and necessary to reach identified goals.
  • Advocate for client and family and perform tasks necessary to remove barriers to necessary, consistent care.
  • Deliver culturally informed services.
  • Meet the utilization requirement identified by your program and supervisor.
  • Adhere to agency risk assessments, policies, protocols, safety questionnaire, on-call procedure, safety planning, and corporate compliance.
  • Ensure that required elements of the ECR/EHR are present, current and in accordance with agency certification and documentation standards.
What You Bring To The Table
  • Minimum of a High School Diploma plus 3 years of experience in a related field, or a Bachelor's degree in a relevant field.
  • Excellent written and communication skills.
  • Ability to work flexible hours, including some evenings and weekends.
  • Initiative and ability to work independently.
  • A willingness and comfort level in providing community-based services to a diverse population of clients.
  • Excellent problem-solving skills and the ability to remain calm and professional in crisis situation.
  • Must be at least 21 years of age; have a valid driver's license and current automobile insurance.
  • Must be able to pass a criminal background check and drug screen.
