Blackbird Health is seeking a Care Navigator to be the central point of contact for established patients, guiding families through our care model and ensuring timely access to therapy and medications. This onsite role blends administrative excellence with compassionate, patient‑focused service.
You will manage scheduling, communications, and provider coordination while upholding privacy standards and collaborating with a supportive team.

Blackbird Health’s Care Navigators are an essential part of our integrated and collaborative care team. This isn't just an administrative role; once a new patient or family has begun care, you serve as a constant source of support and operate as the ultimate owner of their experience—answering questions, providing education, ushering on to next steps, and ensuring each patient is well cared for.
This position is onsite, 5 days a week.
Salary Range: $23 USD - $26 USD
