Care Navigation Coordinator

Job Summary

The Care Navigation Coordinator acts as a liaison and facilitator between MindCare Treatment teams and partner sites, responsible for scheduling, assisting with, and coordinating clinical activities, appointments, information exchange, and consultations across sites and physicians through telemedicine technology. As part of a multidisciplinary team, the Care Navigation Coordinator ensures that high-quality telemedicine services are delivered and properly documented according to company policy and partner site requirements.

Compensation: $25/hour

Schedule: Sat and Sun, 7am CST - 3:30pm CST

Duties/Responsibilities:

  1. Facilitate safe and effective clinical consultations
  2. Work closely with physicians and client staff to ensure the timely and accurate reporting of medical records, schedules, billing, utilization, and outcomes
  3. Manage and coordinate the schedule and appointments, including pre-appointment chart prep and delivery of final encounter notes and supporting documents to the customer
  4. Operate video conferencing equipment, telemedical, and EMR systems
  5. Assist client sites with scheduling to utilize clinical time most efficiently and effectively, and communicate changes in provider availability immediately
  6. Follow appropriate billing and documentation procedures
  7. Assist sites with patient selection and informed consent
  8. Administer screening tools to patients
  9. Conduct and log customer satisfaction checks as required
  10. Perform other duties as required

Required Skills/Abilities:

  • Ability to work independently with minimal supervision
  • Ability to work as part of a team
  • Excellent verbal communication skills with an emphasis on customer service
  • Excellent written communication and documentation skills
  • Ability to work flexible hours as required to meet facility and patient needs
  • High integrity with excellent problem-solving abilities
  • Strong organizational skills and the ability to work on a deadline
  • Proficiency with Microsoft Office Suite and medical office software, including electronic medical records
  • Understanding of and commitment to protecting confidential patient information

Education and Experience:

  • 3 to 5+ years of experience within a behavioral health facility or practice focused on psychiatric/mental health treatment, or equivalent
  • 2+ years experience in practice administration, including scheduling, record keeping, billing, etc.
  • Experience with telehealth technology, including video teleconference technology
  • Knowledge of psychiatric medicines and acute psychiatric service line is strongly preferred
