Position Purpose

Performs care management duties to assess, plan, and coordinate medical and supporting services across the continuum of care for post-discharge members, promoting quality and cost-effective care. Responsibilities include medication review for pre-admission and post-discharge reconciliation, and working with care management and coordination teams to identify transition support services.

Responsibilities
  1. Evaluate member needs by completing post-discharge assessments for members transitioning from healthcare facilities.
  2. Review medications and perform reconciliation between pre-admission and post-discharge medications.
  3. Develop care/service plans and collaborate with discharge planners, providers, specialists, and interdisciplinary teams to support member transition and discharge needs.
  4. Assess current health status, resource needs, services, and treatment plans, providing appropriate interventions.
  5. Facilitate transition into active care management based on member needs.
  6. Provide or facilitate education and resource materials to members, caregivers, and providers to promote wellness activities.
  7. Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical resources as needed.
  8. Perform outreach via phone, digital means, home visits, or other sites to assess needs and collaborate with resources.
  9. Maintain all member information and care management activities in compliance with regulations.
  10. Provide feedback to leadership to improve care and quality delivery cost-effectively.
  11. Perform other duties as assigned by leadership to meet business needs.
Qualifications
  • Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing with 2–4 years of related experience.
  • License/Certification: LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required.
Additional Details

Pay Range: $55,100.00 - $99,000.00 per year
