Job description

LOCATION: Remote - must live in or near Stokes County, North Carolina. The position must live in North Carolina or within 40 miles of the North Carolina border. This position requires travel.

GENERAL STATEMENT OF JOB

The Care Manager is responsible for providing proactive intervention and care coordination to eligible Vaya Health members. The role involves assessment, multidisciplinary care planning, linkage, and coordination across various health domains, supporting transitions, and preventing unnecessary hospitalizations. This is a mobile position working in members' communities and involves collaboration with members, families, providers, and community stakeholders.

ESSENTIAL JOB FUNCTIONS

Assessment, Care Planning, and Interdisciplinary Care Team:

  • Conduct comprehensive health risk assessments and develop person-centered care plans.
  • Link members to appropriate services across health and social domains.
  • Participate in care team meetings and support care plan development and updates.
  • Support transition and diversion efforts, crisis planning, and coordination with providers.

Collaboration, Coordination, Documentation:

  • Manage multidisciplinary team meetings and collaborate with community stakeholders.
  • Ensure proper documentation and compliance with policies and regulations.
  • Support quality assurance and safety protocols.

KNOWLEDGE, SKILL & ABILITIES

  • Effective communication, relationship-building, and organizational skills.
  • Proficiency in Microsoft Office and care management systems.
  • Knowledge of mental health, substance use, I/DD, TBI services, and Medicaid programs.

QUALIFICATIONS & EDUCATION REQUIREMENTS

  • Bachelor's degree in a related field; experience with mental health and LTSS preferred.
  • Licensure as required (e.g., RN license if applicable).

PHYSICAL REQUIREMENTS

  • Visual acuity, mobility, and ability to sit and drive for extended periods.

RESIDENCY REQUIREMENT: Must reside in North Carolina or within 40 miles of the border.

SALARY: Depending on qualifications and experience. Non-exempt position with overtime eligibility.
