Care Manager - Social Worker, Monday-Friday, Day Shift

Care Manager - Social Worker, Monday-Friday, Day Shift

Apply at GBMC Hospital. This full-time position in the Continuing Care Department involves assessing and intervening to ensure seamless continuation of patient care post-discharge. The social worker will work across various units, including medical, surgical, ICU, OB/Maternity, pediatric, and Emergency Department, collaborating closely with physicians, nurses, and other practitioners.

  1. Interview patients and families to identify needs and develop care plans.
  2. Assess psychosocial needs and provide counseling to promote coping and treatment adherence.
  3. Educate patients and families about community resources, rights, responsibilities, advance directives, end-of-life issues, health insurance, and healthcare navigation.
  4. Coordinate complex care plans involving home health, social services, rehabilitation, and mental health providers.
  5. Advocate for patients to receive necessary resources and services.
  6. Participate in department and hospital initiatives for process improvement and patient care enhancement.
Qualifications
  • Masters’ in Social Work or equivalent experience/certification.
  • Licensed in Maryland as LMSW or LCSW.
  • 1-3 years of hospital or healthcare social work experience, preferably in discharge planning.
  • Skills include cultural competency, computer literacy, interview and assessment skills, counseling, and motivational interviewing.
  • Ability to work in a fast-paced environment and engage with multidisciplinary teams.
Additional Information

Salary Range: $56,785.43 - $90,856.69. Final salary depends on qualifications and experience.
