A leading health organization is seeking a Care Manager to provide proactive care coordination and support to members in North Carolina. This role involves conducting assessments, developing care plans, and collaborating with interdisciplinary teams to ensure comprehensive member support. Ideal candidates will have a Bachelor's degree and strong communication skills, with a focus on mental health and Medicaid programs.
LOCATION: Remote - must live in or near Rowan County, North Carolina. The position must live in North Carolina or within 40 miles of the North Carolina border. This position requires travel.
GENERAL STATEMENT OF JOB
The Care Manager is responsible for providing proactive intervention and care coordination to eligible Vaya Health members to ensure appropriate assessment and services. The Care Manager collaborates with members and care teams to address care gaps across various health domains, including mental health, substance use, intellectual/developmental disabilities, physical health, pharmacy, LTSS, and unmet resource needs. This role involves supporting transitions from facilities, ensuring follow-up, and working in diverse locations, including members' homes. Essential functions include:
ESSENTIAL JOB FUNCTIONS
Includes assessment, care planning, collaboration, documentation, and ensuring compliance with all standards and regulations. The Care Manager supports members' needs through assessment, care planning, team coordination, and advocacy, ensuring all documentation is accurate and timely.
KNOWLEDGE, SKILL & ABILITIES
Requires excellent communication, interpersonal skills, attention to detail, problem-solving abilities, proficiency in Microsoft Office and Vaya systems, and knowledge of mental health and Medicaid programs. Ability to work independently and as part of a team, and to adapt to changing priorities, is essential.
QUALIFICATIONS & EDUCATION
Bachelor's degree in a related field is required, with specific experience depending on the population served (mental health, LTSS, I/DD, TBI). Licensure in nursing (RN) and North Carolina licensure are preferred.
PHYSICAL REQUIREMENTS
RESIDENCY REQUIREMENT: Must reside in North Carolina or within 40 miles of its border.
SALARY: Based on qualifications and experience. Non-exempt, eligible for overtime.
