We are looking for a committed and empathetic individual to join our team as a Care Manager. This vital role involves coordinating care for adults with behavioral health needs alongside their medical treatment. The Care Manager plays a crucial role in achieving company goals of improving patient health outcomes, reducing avoidable emergency department and inpatient utilization and ensuring seamless transitions in care from facility to home to prevent readmissions.
The ideal candidate will have a solid background in nursing with a focus on medical and behavioral health and possess outstanding communication and organizational abilities.
