Providence Health and Services Rio Dell, CA, is seeking a Care Manager RN at Redwood Memorial Hospital. This Per Diem, 8-hour Day shift role supports complex patient care across the continuum.

The RN Care Manager plans and coordinates care, communicates with medical staff and outside resources, and ensures utilization meets clinical and financial standards while delivering compassionate, patient-centered care.

Qualifications

• Associate's Degree in Nursing and California RN license required.
• 2 years acute care experience in a Medical Surgical or Inpatient setting.
• 1 year experience in care management or utilization review or completion of TIPS/Case Management Orientation Program.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Plan, coordinate, negotiate, procure and manage care for complex patients and families.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate resource utilization.
• Support a patient-focused, whole-person care approach in alignment with clinical and financial standards.

The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. Communicates on a daily basis with medical staff, hospital personnel and outside resources to ensure the appropriate utilization of resources according to clinical and financial standards.

Required Qualifications:

• Associate's Degree in Associate's Degree in Nursing or a nursing diploma upon hire.
• Upon hire: California Registered Nurse License
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire.

Preferred Qualifications:

• Bachelor's Degree in Bachelor's Degree in Nursing or higher within 3 years of hire.
