Job summary

Providence's NorCal HealthConnect is seeking a Care Manager RN to coordinate care for a caseload of patients at Petaluma Valley Hospital in Petaluma, CA. This per diem role involves establishing a continuum of care and acting as a liaison to facilitate transitions from acute to next level of care, with 8-hour day shifts.

The ideal candidate holds California RN licensure, with 2 years of acute care or similar experience; a Bachelor's degree in nursing is preferred within 3 years of hire, and CCM

Qualifications

• California Registered Nurse license at hire.
• 2 years of acute care clinical or comparable experience.
• Bachelor's degree in nursing preferred within 3 years of hire date.
• CCM certification would be advantageous.

Responsibilities

• Document, report outcomes, and liaise with patients.

Job description

Care Manager RN at Petaluma Valley Hospital in Petaluma, CA. This position is Per Diem and will work 8 hour, Day shifts.

Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. The Case Manager will act as a liaison to facilitate a smooth transition for the patient from acute to the next level of care.

Required Qualifications:

• Upon hire: California Registered Nurse License
• 2 years of Acute care clinical or comparative experience.

Preferred Qualifications:

• Bachelor's degree in nursing. Required within 3 years of hire date
• Upon hire: CCM
• 3 years of experience in community health nursing or case management.
